Urinary iron excretion in thalassaemia after desferri-

oxamine administration. Urinary iron excretion induced with desferrioxamine (DFA) was estimated in 26 children with thalassaemia major. Four separate 24-hour urine collections were made-during a baseline period, after intramuscular injection of DFA 1 g, during blood transfusion with DFA 0 5 g/unit transfused blood, and during the post-transfusion period. Urinary iron, plasma iron, and total iron-binding capacity were estimated by atomic absorption spectrophotometry. Urinary iron excretion in the 24 hours after 1 g DFA ranged from 1 1 to 23 mg/M2 surface area compared with 0 * 1 to 1 * 6 mg/M2 during the baseline period. A positive correlation was obtained between DFA-induced urinary iron excretion and the amount of blood transfused, plasma iron level, and size of the spleen. Splenectomized patients excreted less iron after DFA than those who were not splenectomized. DFA-induced urinary iron was measured before and after splenectomy in 6 patients. In 5 of the 6 patients a drop in iron excretion was observed and the analysis of the 6 pairs of results indicated that splenectomy produced a decrease in DFA-induced urinary iron excretion. These findings indicate that the enlarged spleen in thalassaemia is a major source of iron chelated by DFA. It is suggested that treatment with DFA in the presence of a large spleen may be ineffective in removing excess iron from the myocardium and liver.

متن کاملEarly iron overload in beta-thalassaemia major: when to start chelation therapy?
Twenty-eight children with beta-thalassaemia major aged between 11 and 48 months were given intensive transfusions. Serum iron, transferrin saturation, serum ferritin, non-transferrin iron, and subcutaneous desferrioxamine-induced urinary iron excretion were measured. The results showed that even children with a limited number of transfusions had severe iron overload as indicated, in particular...
متن کاملIntensive iron-chelation therapy with desferrioxamine in iron-loading anaemias.
1. Urinary iron excretion after desferrioxamine has been examined in nine patinets with different iron-loading anaemias. Particular attention has been paid to individual variation in response and the kinetics of iron removal in order to determine the most efficient and convenient method of administration. 2. Twelve-hour subcutaneous infusions of desferrioxamine were comparable with intravenous ...
متن کاملUrinary beta-aminoisobutyric acid excretion in thalassaemia.
The quantity of beta-aminoisobutyric acid (BAIB) excreted in the urine of patients with an intact spleen suffering from thalassaemia major appears to be proportional to the number of the circulating normoblasts and inversely proportional to the haemoglobin level. After splenectomy only minute amounts of BAIB are excreted. Transfusion constantly, but temporarily, reduces urinary excretion of bet...
متن کاملThe Comparison of Efficacy of Original Brand Deferoxamine with Generic Iranian Made Deferoxamine in Urinary Iron Excretion in Patients with Thalassemia Major
Background: Deferoxamine mesylate is still the conventional and well-known iron chelator for patients with thalassemia major. However, due to some marketing issues the well-known original brand, Desferal®, produced by Novartis Pharmaceuticals Company is not as available as before. The generic brands of Deferoxamine have been introduced in many parts of the world including Iran; however, they ar...
